What are the conditons of La Illaha ILLAllaah Wa Muhammadur Rasoolulah? How many are there? What is the last Ayat revealed and recorded from The Prophet Sallalahu Alayhis was Salaam? What are the conditions of the Shahadah and how many are there?

I'll try and help by starting with the conditions of the Shahadah.
There are seven conditions.

They are:
1. Knowledge
2. Certainty
3. Sincerity
4. Truthfulness
5. Love
6. Submission
7. Acceptance

I have also read that Shaikh bin Baz rahimahullah had mentioned one more, which is to reject all Taaghoot.

1. Knowledge (vs. ignorance)
2. Certainty (vs. doubt)
3. Acceptance (vs. rejection)
4. Submission (vs. negligence)
5. Truthfulness (vs. lying)

7. Love for it and its people and disassociation from those who oppose it
